The Ohio Department of Aging and Ohio's aging network operate the PASSPORT program. The program helps Medicaid-eligible older Ohioans get the long-term services and supports they need to stay in their homes or other community settings, rather than enter nursing homes.
PASSPORT includes a screening to determine eligibility and care needs and provide information about long-term care options available. It also includes home care coordinated and supported by a case manager. Services are offered by providers in your community and consumer-directed care options are available.
